OATHTAKING is done on the same
date as scheduled for your registration from 8:00 am – 4:30 pm.
Registrants are required to wear
their GALA UNIFORM during the registration.
NOTE: TO ALL PASSERS, please
bring photocopy of NSO BIRTH CERTIFICATE with original copy, photocopy of
NURSING APPLICATION FORM ( NAF ) and
photocopy of PERMANENT EXAMINATION REGISTRATION RECORD CARD ( PERRC ).
You may get your oath form at the
PRC Office or you can download it. You
may secure the requirements INSIDE the PRC OFFICE. For those who cannot register on their
scheduled date, please register starting March 16, 2012 onwards.
