- Payment: Registration Fee – PhP 600.00 / Annual Fee – PhP 450.00
- Loose Documentary Stamp
STEPS IN REGISTRATION:
- Download & print OATH FORM from www.prclucena.weebly.com
- Submit the properly filled out Oath Form at window 9 for assessment of fees
- Pay registration/annual fee at Window 17
- Secure documentary stamp at the Photo Booth (in front of Window 17)
- Pay OATH TAKING TICKET at the PRB Representative (Photo Booth)
- Submit OATH FORM after payment at Window 10
- Wait to be called at Window 8 to sign the Registry Sheet
- After registration, receive your claim slip, come on your scheduled date of release
For those who want to apply for CERTIFICATION of PASSING, RATING and GOOD STANDING:
9. Secure ACTION SHEET from the Public Assistance and Complaints Desk
10. Pay certification fee at Window 17 (PhP75.00 per copy)
11. Secure documentary stamp the Photo Booth
12. Submit Action Sheet, Official Receipt & photocopied claim slip at Window 7
Note: Please bring your Notice of Admission / Application Stub for verification
