- Duly accomplished oath form and board certificate form
- 2 pcs. colored passport size or 2 x 2 picture, white background with complete name tag
- 1 pc. 1 x 1 colored picture, white background with complete name tag
- Recent Cedula
- Documentary stamp
- Initial registration fee of Php 600.00 and annual registration fee of Php 450.00 for 2012-2015
** Application stub / Notice of admission (for identification)
** Black ink regular ballpen (for filling out the registry)
