- Recent Cedula
- Documentary stamp for the oath form
- Initial registration fee of Php 600.00 and annual registration fee of Php 450.00 for 2011-2014
** Application stub / Notice of admission (for identification)
** Black ink regular ballpen (for filling out the registry)
