Schedule of Registration for Lucena Passers of LET March 2012 Licensure Examination for Teachers

Professional Regulation Commission Lucena Regional Office announces the schedule of registration for passers of LET March 2012 Licensure Examination for Teachers also known as  March 2012 Teachers Board Exam passers. This announcement are only for the passers from Region IV / Lucena officially  posted by the PRC. 

Registration started on April 26 to June 11, 2012 and onwards

Requirements:

• 3 pieces passport size colored picture in white background with complete name tag (Last Name, First name, Middle Name) e.g. GAGALAC, ARNEL ROSARIA
• Duly accomplished oath form and board certificate form
• 1 pc. 1 x 1 colored picture, white background (same shot)
• 2012 Community Tax Certificate (Cedula)
• Payment: Registration fee of Php 600.00 / Annual Fee of Php 450.00 
• Loose Documentary stamp

  Steps in Registration:       

1. Present NOTICE OF ADMISSION / APPLICATION STUB or any VALID ID at Window 8 (for verification of LET rating) to secure OATH FORM.
2. Check your NAME and DATE OF BIRTH from your verification slip, let us know if there is any discrepancy.
3. Fill-out the oath form properly.
4. Pay annual/registration fee at Window 3;
5. Secure documentary stamp at the Customer Service Center
6. Pay OATH TAKING TICKET at PRB Representative
7. Submit the properly filled-out OATH FORM at Window 7.
8. Wait to be called at Window 6 or 7 for the registration proper.
9. After registration, receive your claim slip. (Check the presence of the following - registration number & date serial number)

For those who want to apply for CERTIFICATION of PASSING and RATING:

1. Secure ACTION SHEET from the guard
2. Pay certification fee at Window 3 (Php75.00 per copy)
3. Secure documentary stamp at the  Customer Service Center.
4. Submit action sheet, official receipt & photocopied verification slip and claim slip at window 7.

Note: Please bring your Notice of Admission / Application Stub for verification.

Time:    8:00A.M.

Venue:  SACRED HEART COLLEGE GYMNASIUM

Attire:   Smart Casual / Semi - Formal / Sunday Dress
